When your child turns three it is a big transition, not just in learning and development, but in support services. This information covers the transition process and options that may be available to your child when Early Start ends.
The Transition Meeting
How: Your child's regional center service coordinator must schedule a transition meeting. When: At least 90 days before your child's 3rd birthday. Where: At a time and place that is convenient for you and the other members of the team. Who must attend: You (the parent), regional center service coordinator and a representative from the local education agency. Who may also attend: Any family members, friends and/or professionals who may provide helpful information.
What will be discussed?
The transition meeting is held to design a plan that outlines the steps everyone will follow over the next few months to help make your child's transition a smooth one.
Be prepared to:
share your hopes for your child
receive information about you and child's rights
ask about how eligibility will be determined
ask what services are available and in what ways can they be provided
ask what placements are possible
ask how to visit potential placements
provide the local district with copies of final reports of early intervention providers
request a blank copy of the local district IEP form
ask what happens to your child during summer if transition occurs in spring/summer
What should the transition plan include?
who, what, where, when and how
specific steps to be taken to ensure a smooth transition
specific individuals responsible for taking those steps
timeline for completion of those steps
specified time for the next meeting
Programs Available to all 3 Year Olds
Programs may vary slightly from region to region and are typically available to all children. Some may have enrollment fees, financial or other eligibility criteria. Your Service Coordinator will be able to assist you in finding local resources.
